Services
Helping patients and their families to overcome disabling injuries and diseases is the challenge we face every day. You, too, will face many challenges during your stay with us. We are here to share your challenges, to help you return to a more independent way of life and to reach your highest potential. We understand your challenges are unique and your needs are personal. Therefore, your rehabilitation program will be tailored to your individual needs.
Our team of skilled medical and rehabilitation specialists will be by your side every step of the way to help you reach your goals. Because we see rehabilitation as a family achievement, we encourage participation from family and/or your significant other. We’ll make special arrangements for them to observe and learn more about the therapy being given to you. Family support and educational group sessions are held weekly.
The goal of our comprehensive inpatient program is to improve the functional abilities of the patients served and their capability to function in their various environments. The program is applicable to those who have severe functional limitations of recent onset, recent regression or progression, and to those who have not had prior exposure to rehabilitation.
The Team Approach
Carolinas Rehabilitation Hospital utilizes a team approach toward patient care and sees you, the patient, as the most important member of the rehabilitation team. Your participation is vital. Your family and/or significant other are also key elements in the recovery process.
By understanding the implications of your disability, treatment and recovery goals, we can help you make a smooth transition back to the activities of daily living. Helping you through the rehabilitation process will be a team of specialists, who are experienced, talented professionals working toward a common goal – your recovery. You will meet the members of your team during the first few days of the evaluation process to design a rehabilitation program appropriate to your needs. The team will meet once a week, or more often if necessary, to evaluate your progress.
Members of the Rehabilitation Team include:
- Attending Physician
- Medical Director/Physiatrist
- Nurse Liaison
- Occupational Therapist
- Other Consulting Physicians
- Patient Assessment Instrument (PAI) Coordinator
- Physical Therapist
- Psychologist
- Registered Dietitian
- Rehabilitation Nurse
- Social Worker/Case Manager
- Speech Language Pathologist
- Support Staff
Patient/Family Conference
Patient/Family Conferences will be held following the Team Conference. The date of this conference will be confirmed with you and your family by the Social Worker. At the conference, you will have a chance to meet with the physician. Each session will last 5 to 10 minutes. If at any time you have a problem with the scheduling of your patient/family conference, please contact the Social Worker. At this conference, the patient/family will be informed of goals, progress and tentative discharge date.

A nurse and/or the Social Worker will call a priest, minister or rabbi at the request of the patient and/or the family. Pastoral services are available to all patients, family members or employees through the hospital chaplain.
